Describe what you saw, where you saw it and whether any cattle were involved. Do not include your address: they already have it, and we don't want it.
Observe from a distance. Do not approach craft, beings, circles or Commodores.

Your report has been placed in the Tupperware for assessment. If the Men in Beige arrive in the next 24 hours, which they will, do not mention this form.
Please do not file a second report to confirm the first. The first is also fine.
What happens next
- Warren reads it on the verandah.
- Dr Faraday assigns a Faraday Unit value to any light described.
- Noreen reads it and writes "or a ute" in the margin where appropriate.
- It is filed as CONFIRMED, UNVERIFIED BUT TRUE or DEVELOPING, and a pin is added to the map.
- You are invited to the Tuesday group. Bring a plate.
Plain language: this form sends nothing anywhere. The acknowledgement is theatre. No report is transmitted, stored, logged or read by anyone, which, as the Collective has noted elsewhere, is more than the Bureau can say.
